We are seeking an empathetic and professional Social Worker to join our dedicated team in enhancing the lives of our elderly residents and their families.

Job Description:

As a Social Worker at Cliffside Nursing Home, you will play a vital role in providing emotional support, counseling, and assistance with social services to residents and their families. You will be responsible for developing care plans, advocating for resident needs, and helping families navigate the challenges of long-term care.

Responsibilities :

• Assess the social, emotional, and environmental needs of residents and their families.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Provide emotional support, counseling, and crisis intervention for residents and their families.
• Assist with the transition process for new residents and help families understand available resources.
• Advocate for residents' rights and ensure they are treated with dignity and respect.
• Help residents connect with community resources and support services as needed.
• Maintain accurate documentation of social work interventions and resident progress.
• Attend care plan meetings and contribute to multidisciplinary team discussions.
• Ensure compliance with state and federal regulations related to social work services.

Requirements :

• Must be bilingual in English and Chinese (Mandarin or Cantonese).
• Previous experience in long-term care or healthcare settings is preferred but not required.
• Strong communication, interpersonal, and organizational skills.
• Empathy, patience, and a passion for helping others.
• Ability to work independently and as part of a team.

Compensation :
